You can either send forms by mail to BRMS or fill them online.

Mail Forms: Download the respective forms and mail them to 560 N Nimitz Hwy STE 209, Honolulu, HI 96817.

1. All participants must complete the Child Care Affidavit, banking information, and select ONE Benefit Designation, not both.

   • • Informal Care Benefit Designation Form
     • Formal Care Benefit Designation Form
2. After you are awarded a slot, complete the corresponding forms to the benefit you applied for, not both:

   • INFORMAL:

     • Informal Proof of Care Form – Caregiver fills out

   • FORMAL:

     • Formal Care Provider Form – Provider fills out (it is attached in the email)
       
     • Formal Proof of Payment Form – Participant fills out

Note: Once the forms are submitted, you will be notified if you are awarded a slot or put on a waiting list.
